Unlimited GP SmartCare with R100 co-pay- Network Providers must be used, 2 out of Network consultations per year for GP's per person thereafter 40% upfront co-payment and R100 per consultation; 1 Specialist Visit per year; Prescibed Medication: R610 per family per annum. R10 upfront co-payment per medicine. Over-the-counter medication: R385 per family. Limited to R110 per script. Basic Dentistry: 1 Examination with R150 upfront co-payment per beneficiary. Subject to Dental Network. Optical Benefit: R940 per beneficiary per 24 months. 1 Eye Test per beneficiary per year; R200 per beneficiary per year for Readers. All of the above benefits must be obtained through the Optometry Network and has a R100 upfront co-payment. |
